Sixty five year old farmer presented with asymptomatic scaly plaques on dorsa of hands. Biopsy findings are shown in figures. What is your diagnosis?

The diagnosis is acantholytic actinic keratosis. Note resemblance to pemphigus vulgaris due to suprabasal acantholysis. Abundant solar elastosis (seen as dark blue material) is seen in mid and deep dermis. High power shows dyskeratosis and nuclear atypia. |
